picKey goals of disease management programs are to encourage members toward healthy behaviors and to drive improvement in clinical outcomes and utilization. The purpose of this study was to evaluate the impact of home-monitoring as part of heart failure (HF) disease management programs. Specifically, the ability of home-monitoring to promote improved HF management, particularly reduced fluid retention, for members with HF and members with HF and diabetes mellitus (DM) was assessed.
