Healthways Diabetes Services for Hospitals currently helps hospitals deliver consistent, high-quality care to more than 100,000 patients with diabetes annually. We work closely with hospitals to develop and implement seamless integration of inpatient and outpatient programming designed to meet the special needs of patients with diabetes and their physicians. Healthways supports hospitals by providing either on-site diabetes service line management or simply consulting support in the analysis, design, implementation, and integration of specialized diabetes services such as inpatient hyperglycemia management.
Inpatient and Outpatient Services
Diabetes affects every aspect of patient care – from the emergency department to your surgical suite. Healthways provides comprehensive inpatient and outpatient programming designed to meet the special needs of patients with diabetes. We provide the strategies, processes, tools and resources to help hospitals implement concurrent and systematic inpatient interventions, outpatient self-management skills development and outcomes management. We are fully experienced in supporting the many complications of diabetes, such as heart disease, stroke, blindness, kidney disease and amputations.
Inpatient Services to Improve Glycemic Management
Our inpatient services can help hospitals provide effective and efficient diabetes care through evidence-based standards and current best practices. Our support helps physicians and nurses keep inpatients at normal glycemic levels throughout their stay – regardless of the reason for admission – using an on-site support staff of dedicated, specially trained nurses and case managers to assist with clinical interventions.
Outpatient Self-Management
The Healthways outpatient self-management program is a 12-month education program designed to help individuals with diabetes improve their health status and quality of life, as well as reduce the risks for diabetes-related complications. Our outpatient program is designed to meet the standards for recognition from the American Diabetes Association, required for Medicare reimbursement.
