The Health Improvement Program for Care Management is a focused approach to providing support for people living with complex medical conditions. This program provides individualized support and education to you and your doctor. If eligible for this program, you and any dependent family members can participate at no additional cost to you as part of your employer’s benefits package. You’ll receive valuable support to help you:
