If you choose to participate in the Health Improvement Program, your health plan and Healthways, Inc., the company administering the program, will have access to your information. Your doctor will also receive information you share with your health plan or Healthways during the time you are in the program.

Yes. All information will be kept confidential by your health plan and Healthways. Your doctor may also receive information from your health plan or Healthways during the time you are in the program.

It is your health plan's and Healthways' policy not to discuss any information about members with anyone but the member and his or her doctor, unless member permission is obtained.

No. The Health Improvement Program is an educational program only and in no way replaces the care you receive from your doctor. You should continue to see your doctor for your regular visits.
